| 内容記述 | In Japan, the AIDS Clinical Center (ACC), part of the International Medical Center of Japan (IMCJ), was established in April 1997. In addition, each prefecture has several registered hospitals for HIV/AIDS therapy. The ACC collaborates with eight regional AIDS centers and 367 hospitals that have HIV/AIDS care facilities. The aim of this structure is to improve the access and delivery of advanced health care services for people infected with HIV/AIDS through out Japan. Following a request from the Plaintiff Group, HIV/AIDS coordinator nurses (CNs) was set up in ACC and eight regional AIDS centers. The purpose of this study is to examine what is brought into HIV-infected patients and the HIV/AIDS medical care system by CNs care. Fifty-one subjects were randomly selected from HIV-infected patients, who received the treatment of HIV/AIDS in ACC and three regional AIDS centers, and agreed to participate in this study. The data were collected through semi-structured interviews, which were recorded on the tape and transcribed. Data were classified from the content of the CNs caring and the evaluation by the KJ method and categorized. They were examined until agreement was obtained between researchers to ensure the reliability and the validity of the analyses. Data from the interviews revealed that, the content of the CNs caring was able to be classified into "Consultation and Counseling", "Offer of information", "Education", "Support of relationship between patients and doctors", "Establishment of supporters" "Cooperatron and adjustment inside and outside of facilities", "Initial education" "Consultation and guidance of taking medicine", and "Connection between home caring and hospital". In addition caring by CNs had brought the patients "Sense of security", "Understanding of sickness and treatment", "Recognition that the patients themselves were subjects of the medical treatment", "Immediate response to the problem in home care" "Appropriate support for needs", and "Economical support". It was also shown that caring by CNs was able to improve patient's self-care ability and to change their attitude for decisionmaking by themselves. Furthermore, the patients became positive about the treatment of HIV/AIDS after caring by CNs. Collectively, CNs attributed the patientoriented medical care of HIV/AIDS to the coordination between public health, medical treatment, and welfare. |
