{"created":"2023-05-15T09:57:45.523728+00:00","id":1600,"links":{},"metadata":{"_buckets":{"deposit":"51591f36-c8f4-4727-b00a-dcb6abb90d2c"},"_deposit":{"created_by":5,"id":"1600","owner":"5","owners":[5],"pid":{"revision_id":0,"type":"depid","value":"1600"},"status":"published"},"_oai":{"id":"oai:miyazaki-u.repo.nii.ac.jp:00001600","sets":["72","72:30"]},"author_link":["9487","9611","8155","7768","9609","9608","7929","8142","8158"],"item_10001_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2013-09","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"118","bibliographicPageStart":"111","bibliographicVolumeNumber":"37","bibliographic_titles":[{"bibliographic_title":"宮崎県医師会医学会誌","bibliographic_titleLang":"ja"},{"bibliographic_title":"The Journal of the Miyazaki Medical Association","bibliographic_titleLang":"en"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"若年（ティーンエイジ:13～19歳）で診断されたIgA腎症患者の臨床病理学的特徴や予後因子の検討は少ない。今回,成人IgA腎症患者との違いを比較・検討した。研究デザインは過去起点コホート研究。対象は1981年からの25年間に当院でIgA腎症と初回診断された365名のうち,腎生検時の血清クレアチニン値が2.0mg/dl以下で,12カ月以上経過観察し得た304名。対象を若年（13～19歳）75例と成人（20歳以上）229例の2群に分け,両群の背景因予を比較した。また,若年群について末期腎不全への進展に寄与する因子を検討した。若年群の64.0%は無症候性血尿/蛋白尿を契機に診断されており,成人の38.4%より有意に多い割合だった。若年群は成人群より有意に診断時の血圧,血清クレアチニン値が低く,1日蛋白尿量が少なく,組織病変の重症度も軽度であった。若年群の10年腎生存率は87.8%で,平均81.9カ月の観察期間中に末期腎不全に至った8例では,初期治療としてレニン・アンジオテンシン系阻害薬やステロイド薬を投与されていなかった。Cox比例ハザードモデルによる多変量解析では,若年群でも診断時の尿蛋白量と病理所見の重症度が有意な予後因子であった。若年者は成人より軽症の段階で診断されている。しかし,若年であっても尿蛋白量が多い例や重度の組織障害を呈する例では末期腎不全へ進展する可能性があり,積極的な治療介入が必要と思われる。","subitem_description_language":"ja","subitem_description_type":"Abstract"},{"subitem_description":"Although several observational studies of childhood or adult IgA nephropathy (IgAN) have been reported, little is known about the clinicopathological features and prognosis of teenagers diagnosed with IgAN. We examined data from 304 patients with IgAN whose serum creatinine (sCr) value at renal biopsy was <2.0 mg/dl and who had been followed up for >12 months. We compared the clinicopathological features of these patients after classification into either teenage (13-19 yr, n=75) or adult (20 yr, n=229) groups. Prognostic factors affecting disease progression in teenage patients were also examined by multivariate analysis. Teenage patients had a significantly lower SBP (mean: 117.9 vs. 128.6 mmHg, respectively ; p<0.001) and sCr (mean, 0.75 vs. 0.87 mg/dl, respectively ; p=0.001) and mild proteinuria (mean, 0.68 vs. 1.41 g/day, respectively ; p<0.001) at diagnosis than adult patients. The number of patients with moderate to severe histological lesions was also significantly lower among teenage compared to adult patients (48% 79%, respectively ; p<0.001). The 10-year renal survival rate of the 75 teenage patients was 87.8%, and 8 of them reached end-stage kidney disease (ESKD) within 81.9 ± 55.1 months. None of these 8 patients had received steroid and/or rennin-angiotensin system inhibitors. The Cox proportional hazards model revealed that severe proteinuria and severe histological damage were independent prognostic factors for ESKD even among teenage patients. Although IgAN in teenage patients can be diagnosed at a relatively early stage, aggressive therapeutic intervention is desirable if severe proteinuria and/or severe histological lesions are evident at
