{"created":"2023-05-15T09:57:34.074656+00:00","id":1390,"links":{},"metadata":{"_buckets":{"deposit":"7bf0a5ea-a1ba-4eac-b603-a53abc7de800"},"_deposit":{"created_by":5,"id":"1390","owner":"5","owners":[5],"pid":{"revision_id":0,"type":"depid","value":"1390"},"status":"published"},"_oai":{"id":"oai:miyazaki-u.repo.nii.ac.jp:00001390","sets":["72","72:30"]},"author_link":["8299","8016","8230","8293","8019","8292","8291","8297"],"item_10001_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2002-12-15","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"3-4 (別冊)","bibliographicPageEnd":"180","bibliographicPageStart":"175","bibliographicVolumeNumber":"48","bibliographic_titles":[{"bibliographic_title":"九州神経精神医学","bibliographic_titleLang":"ja"},{"bibliographic_title":"The Kyushu neuro-psychiatry","bibliographic_titleLang":"en"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"治療抵抗性で約10年間症状が遷延化した老年期うつ病の症例に修正型電気痙攣療法 （modified-ECT : m-ECT）を行い, 臨床症状の著しい改善を示した一症例を報告する｡  症例は70歳の女性, 頭部MRI-CT上で無症候性脳梗塞像を認めたが, 著しい痴呆症状は認めなかった｡  2クール目のm-ECT終了1週間後, 2ヶ月後, 3クール目のm-ECT終了1週間後の計3回SPECTを行った｡  m-ECT終了1週間後に行ったSPECT所見を three dimentional stereotaxic ROI template（3D-SRT）を用いて解析したところ,  主に両側前頭葉・両側頭頂葉並びに海馬に顕著な脳血流量の改善を認めた｡ しかしながら, 症状改善は一過性（約2週間程度）であり易再燃性を示すと同時に, m-ECT直後から両側前頭葉･頭頂葉並びに海馬において著しい血流減少を認めた｡  本症例では無症候性脳梗塞像を認め血管性うつ病という背景病態を有していたが,症状再燃と並行しm-ECT直後から両側前頭葉･両側頭頂葉･海馬において血流減少を呈しやすいという機能的脆弱性も,本症例においてうつ状態が遷延化した経緯の生物学的背景と考えられた。\n\\nA case of intractable senile depression occurring in a 70-year-old female at our hospital is reported based on the analytical findings of 99mTc-HM PAO SPECT performed before and after undergoing modified electroconvulsive therapy （m-ECT）.  The mean cerebral blood flow （mCBF） after m-ECT was remarkably higher than that before m-ECT.  The SPECT findings in this case analyzed using a three dimensional stereotaxic ROI template revealed a remarkable degree of hypoperfusion in the front-parietal cortex and the hippocampus.  These hypoperfused areas all tended to show an improvement in their m CBF after m-ECT.  In addition, the symptoms of a depressive state, hypochondriacal state and obsessive compulsive state in this case all remarkably improved owing to the m-ECT regimen.
